THIS AGREEMENT is made between KELLYS PET GROOMING & SUPPLY and the owner or owners

identified above (collectively the Owner.). The Boarder and the Owner agree as follows:

Boarding Services

1. The Boarder agrees that it will provide boarding services to the Owners animals listed. (collectively

the Animal.) during the term of this Agreement.

Term

2. Kellys Pet Grooming & Supply agrees to provide boarding services to the Animal for a total of _______ days, from:

____________2008 (drop off date) to ____________ , 2008 (pick up date).

3. In the event that the Animals stay is longer than the Term referred to, the Owner

agrees to pay additional daily Boarding Fees.

Payment and Deposits

4. The Owner agrees to pay the applicable daily boarding fees as follows:

1 pet ….. $10.00 per day

2 pet ……$15.00 per day

3 pet ……$20.00 per day

4+ pet ……$ 2.00 additional per day

All payments will be made by cash, check, Visa, MasterCard, or Discover.

5. All Boarding Fees are payable upon pick-up of the Animal.

6. If the Owner is a first-time customer or if the Animal is staying at the Kennel for a term of more than 30 days, Kellys Pet Grooming & Supply may ask for a deposit equal to one-half of the anticipated total Boarding Fees.

Pick-ups and Drop-offs

7. All pick-ups and drop-offs will occur during the business hours unless otherwise previously arranged (Sunday pick ups between 3 pm and 6 pm)

8. In the event that the Owner has not returned to collect the Animal on or before 8:00 p.m. of the scheduled

date of pick-up, the Owner agrees to pay the full days Boarding Fee, unless prior alternate arrangements

have been made with the boarding facility.

9. Please, all dogs must be on-leash and all cats must be contained when being picked up or dropped off.

Health of Animals

10. The Owner represents that the Animal has not been exposed to distemper or rabies or any other infectious diseases or parasites within the last 90 days. The Owner further represents that the Animal has received all required vaccinations and that the Animal is in good health. Any pet with fleas with will be treated at owners expense with Capstar and topical, so as to not spread them to other pets.

11. Kellys Pet grooming & Supply reserves its right to refuse to provide boarding services to any animal.

Unexpected Illness

12. In the event that an Animal becomes ill during its stay at the Boarder, Kellys Pet Grooming & Supply will make all reasonable efforts to immediately contact the Owner.

13. In the event that we are unable to reach the Owner, or if the Owner does not immediately provide

instructions for the care of the Animal to the us, and if the state of the Animals health (in our judgment) requires immediate medical attention, Kellys Pet Grooming & Supply is authorized to make all reasonable efforts to ensure that the Animal receives proper medical attention and may engage the services of a veterinarian of the Boarders own choosing.

4. The Owner agrees that it will pay for all medical treatment received by the Animal, and any other related feel

or disbursements incurred on the Owners behalf, during the Term of the Agreement.

15. The Owner understands and agrees that Kellys Pet Grooming & Supply staff are not trained to recognize or diagnose animal ailments or diseases and the Owner agrees that the Kennel will not be liable for any sickness or ailment that

the Animal develops or manifests during its stay at the Kennel.

Unclaimed Animals

16. In the event that an Owner has not returned to the Kennel to pick-up the Animal within 10 days after the

scheduled pick-up date referred to in paragraph 2 and has not made any other arrangements for payment

and/or the continued boarding of the Animal at the Kennel, the Kennel may, at its sole discretion, sell or

otherwise dispose of the Animal without notice to the Owner.

Liability

17. The Owner understands and agrees that Kennel will not be liable to the Owner nor to any other party for any

liability, any consequential losses, any damages whether in contract, tort or otherwise, or any other claims or

expenses that may be connected with the services provided by the Kennel including, but not limited to, loss

or damage from disease, death, running away, theft, fire and injury to persons, other animals or property.
